Emotional Health and Well Being Projects for Young people living in Wyre and Fylde
September 2014 to March 2015(project may be extended subject to funding)
URPotential are commissioned by Lancashire Young People’s Service to provide Targeted Youth Support under Emotional and Health and Well Being.
The service will work with young people in respect of their emotional health and wellbeing who are Just Coping or showing signs of Struggling to Cope, (Level 3) to build their capability and capacity for them to manage challenges/difficulties safely within their home environment, and the local community and enable them to Thrive (Thriving as defined by the Lancashire Continuum of Need).
URPotential will deliver targeted programmes in each of the areas for vulnerable individual young people aged 12 – 19 (up to age 25 for young people with learning difficulties and disabilities (LDD) aimed at ensuring that young people at greatest risk of multiple disadvantages get the best start to their adult life. All young people must be referred to the project by completing the attached referral form, we will accept self referrals.

Our team will be happy to attend a staff meeting to give a presentation about the project. We do encourage you to refer young people to the project and we will work alongside organisations in partnership to support young people in their Emotional Health and Well Being needs.
